The forms listed below must be on file in the Main Office for an athlete to be eligible to participate in any sport tryouts or practices.
KSHSAA PPE-Physical Dated after May 1 - requires Doctor, Athlete and Parent/Guardian signatures |

USD497 Insurance Compliance Requires parent/guardian signatures notarization and dated after May 1 |
|
USD497 Code of Conduct Form Once signed it is valid until graduation - requires athlete and parent signatures |

Tryout/Participation Agreement Dated after May 1- Requires athlete and parent signatures |
|
-Turn all forms in to the Main Office not to the coaches.
-A current list of athletes with all forms on file is posted outside of the athletic office the week prior to tryouts.
-If an athlete's name is not on the list they have incomplete or missing forms and need to check with the athletic office.
